This study aimed to compare the effectiveness of positive therapy and a self-compassion-based family empowerment model on treatment adherence, self-care, quality of life, and glycosylated hemoglobin in women with type 2 diabetes. The study was a quasi-experimental design with a pre-test, post-test, and three-month follow-up with a control group. The study population included women with type 2 diabetes, 45 of whom were selected through convenience sampling and randomly assigned to three groups of 15 including positive therapy, self-compassion-based family empowerment, and control. The experimental groups received intervention sessions, and the control group was placed on a waiting list. Data were collected using instruments measuring treatment adherence, self-care, quality of life, and glycosylated hemoglobin index, and analyzed using repeated measures analysis of variance and paired comparisons. The findings showed that the effects of group, time, and the interaction of time and group on treatment adherence, self-care, quality of life, and glycosylated hemoglobin were significant (P < 0.001). Also, both interventions increased treatment adherence, self-care, and quality of life and decreased glycosylated hemoglobin compared to the control group, and most of these changes were maintained at the follow-up stage. Overall, both approaches were effective in improving psychological and self-care indicators and diabetes control, and the self-compassion-based family empowerment model was more effective in some indicators.
